What each part of a SOAP note means
SOAP stands for Subjective, Objective, Assessment, and Plan. Keeping these four parts distinct is what makes a note consistent and easy to review — here is what belongs in each.
- Subjective
- What the child, parent, or caregiver reports — mood, concerns, and any changes at home or school since the last session. This is reported information, not what you measured.
- Objective
- What you directly observed and measured during the session — the activities used, the level of prompting or support, and quantifiable performance (e.g. "4 of 6 trials with moderate verbal cues").
- Assessment
- Your clinical interpretation of the session — progress toward goals, factors that helped or limited performance, and the reasoning that links the objective data to your conclusions.
- Plan
- What happens next — the focus for the next session, any home program, session frequency, and whether goals should be adjusted.
