What is a SOAP note in pediatric therapy?
A SOAP note is the standard format for recording a single therapy session. SOAP stands for Subjective (what the child or parent reported), Objective (what the therapist observed and measured, with data), Assessment (the clinical interpretation of that data against the child’s goals), and Plan (what happens next). Keeping measured facts in the Objective section separate from interpretation in the Assessment section is what makes a SOAP note clear and defensible.
How to write a SOAP noteWhat should an OT progress note include?
A pediatric occupational therapy progress note should include the functional goal being addressed (such as independent feeding or in-hand manipulation), objective performance data (level of assistance, number of trials, duration of tolerance, quality of movement), relevant sensory and behavioural observations, a clinical interpretation of progress toward the goal, and the plan for the next session including any home or classroom carryover. As with any note, measured observations should be kept separate from interpretation.

An IEP, or Individualised Education Plan, is a written plan that sets specific, measurable goals for a child with additional needs and describes the support and interventions to help them reach those goals. It is typically developed collaboratively by the professionals working with the child — special educators and therapists such as OTs and speech-language pathologists — together with the family, and reviewed periodically as the child progresses. The measurable goals in an IEP are what individual session notes then track against.
